What is ComB?

Comprehensive Behavioral Therapy (ComB) is a highly effective, evidence-based treatment for Body-Focused Repetitive Behaviors (BFRBs) such as hair pulling (trichotillomania), skin picking (excoriation disorder), nail biting, and more. These behaviors are often automatic, emotionally driven, and difficult to control—causing both physical harm and emotional distress.

ComB therapy helps individuals understand the unique patterns behind their BFRB and builds a personalized plan to interrupt those patterns. By exploring the internal and external factors that drive the behavior, ComB empowers individuals to regain a sense of control, reduce urges, and develop healthier coping strategies.

How Does ComB Therapy Work?

Identify the Five Functional Domains

ComB therapy begins by helping individuals explore five areas that contribute to BFRBs: sensory, cognitive, emotional, motor, and environmental factors. This step brings clarity to what triggers and maintains the behavior.

Personalize a Behavior Change Plan

Once patterns are identified, the therapist works with the client to develop individualized strategies that target their specific BFRB triggers. This may include tools like fidget objects, habit blockers, self-monitoring techniques, and emotional regulation skills.

Practice, Adjust,
& Strengthen

ComB therapy is flexible and responsive. Clients actively practice their strategies, review what’s working, and adjust as needed. Over time, this helps reduce the frequency and intensity of BFRBs and builds long-term behavioral change.
